What do I need to bring with me on my first visit?

On your first visit with one of our physicians, please make sure to bring the following items:

  • new patient form
  • medical record request form
  • insurance information
  • current photo id

How do I transfer my medical records and films from another physician?

If you are a patient who needs to have copies of medical records from a physician sent to a SpinePain & Orthopedic Injury Center physician, you can print out the:

Medical Record Request Form.

Once you have printed the document, filled it out and signed it, please take it to your physician’s office or fax it to (727) 210 – 0880.
